 Routine Eye Examination & Refraction

1. Medicare: Not Covered. Patient is responsible for payment.

2. Tenncare Select: Covered for patients under 21 years of age. Not covered for patients 21 years of age and above.

3. Commercial Plans: Varies by plan.


Medical Eye Services

1. Medicare: Covered for medical necessity

2. Tenncare Select: Covered for patients under 21 years of age. For patients 21 years of age and above Referral / Authorization is required.

3. Commercial Plans: Varies by plan.


Glasses

1. Medicare: Patient is responsible for payment. Medicare may provide some post-cataract coverage.

2. Tenncare Select: Covered for patients under 21 years of age. Not covered for patients 21 years of age and above.

Contact Lenses
