Your whole person care navigation team
This solution is composed of a nurse navigator who is a certified health coach, tobacco treatment specialist and lifestyle medicine practitioner, as well as a licensed mental health professional. These roles work together as a team to provide whole person care. Specifically:
Nurse Navigator:
Helps employees find care, answer questions and guides to helpful resources and appropriate clinical care
Educates about disease treatment options and lifestyle medicine programs that focus in on the six pillars of whole person health: nutrition, physical activity, stress management, restorative sleep, social connection and avoidance of risky substances
Triages complaints to the right level of care and most convenient location
Facilitates referrals to primary, specialty and acute care
Provides health coaching, goal setting and follow-ups to support health goals
Licensed mental health professional:
Serves as direct connection to community therapists, community resources and employer-sponsored benefits, such as an Employee Assistance Program (EAP)
Provides mental and emotional health support and guidance
Offers short-term cognitive behavioral therapy
Identifies and facilitates referrals to address needs
Delivers onsite/virtual educational sessions and group sessions
