Refer patients to VUMC specialists and securely access records through our online platform

As your partner in specialty care, Vanderbilt University Medical Center (VUMC) wants to make it as easy as possible for you to refer to our clinicians and remain informed of your patients’ care at VUMC. We’ve recently introduced new referral methods, including a dedicated online platform for viewing your patients’ records and simple, secure online referral forms for many of our programs. As always, our physician outreach department stands ready to assist you with access support and information regarding our clinicians and programs.

Health Connect: Access your patients’ VUMC medical records 24/7

Vanderbilt Health Connect is a web portal that allows referring clinicians to view their patients’ VUMC medical records 24/7. So, when a patient has been discharged from VUMC and has returned to the community practice, clinicians can review and discuss the VUMC medical records with the patient at any time. Clinicians and practice staff can also place an order for a non-urgent referral through Health Connect.

Anyone in a practice may access Health Connect once it is set up and the user has cleared VUMC’s security check point. The online portal requires a signed Site Agreement

Physician Outreach & Connections: Liaisons to help coordinate care

VUMC is committed to assisting peers across the region and throughout the nation with complex cases and specialty patient care. VUMC’s Physician Outreach & Connections group is available to provide support and information for community practices. Liaisons can also help clinics sign up for Health Connect.

Look for “Refer a Patient” on the bottom of the website for quick links to access Health Connect, directories of VUMC physicians, phone numbers to make referrals, a link to the Site Agreement and other resources for referrers.
