What to Bring to your Appointment

• Insurance card
• Films, MRI or X-ray scans (current for the involved body part)
• Previous operation notes (current for the involved body part)

What to Wear to your Appointment

• Doctor visit:  Wear clothing that is easy to remove and put back on. For example, if you have a knee injury, wear shorts.
• Physical therapy / rehabilitation patients: Dress in comfortable clothing that will be appropriate for working up a sweat. Athletic shoes are a good choice.
