For appointments, call (615) 322-2377 between 8 a.m. and 4 p.m. Monday-Friday.

Our office is in the Vanderbilt Clinic, Suite 3501A. From the lobby of The Vanderbilt Clinic, there are overhead signs directing you to Oral and Maxillofacial Surgery. 

Valet parking is available at the front door of The Vanderbilt Clinic. We can provide wheelchair assistance from the entrance to The Vanderbilt Clinic. 

Patient Forms and Resources

What to Bring 

  • Insurance card(s) (medical and dental)
  • List of current medications
  • Any other relevant information about your condition
  • Tests or biopsies relevant to this visit


We cannot use printouts of digital x-rays, so please ask your dentist to send a digital version (jpg) to or send your own digital copy.

If you don't have a recent digital x-ray or high-quality original non-digital x-ray, you will need to have an x-ray made at your appointment. If you recently had an x-ray charged to your dental insurance, your insurance may not approve another and you will be responsible for the cost.

Doctor Referrals 

Your health insurance plan may require a referral from your primary care doctor before you can see a specialist. Please coordinate your referral with your primary care doctor well in advance of your appointment. Call the member services number on your insurance card to check.

Your doctor may fax your referral to us at (615) 343-3747.


We will collect your co-payment or co-insurance on the date of service according to your insurance plan. If your insurance plan will not cover a particular service, we will request payment at the time of your appointment. Our staff can provide an estimate of charges.
