What is medication reconciliation? Medication reconciliation is the process of comparing the list we have of your medications to what you actually take at home. We can then add medications that we were unaware that you were taking or take medications off of the list that you no longer take.

Why don’t we have an up-to-date list? Many of our patients see other physicians for other health problems. These physicians may provide medications that we might not know about unless brought into the clinic for review. Another situation is when a patient may have to seek treatment at a hospital and they are discharged with new medications that we are unaware of. Many people also take vitamins, herbal supplements and over the counter medications which we also need to know about.

Why is this important?
By having an up to date medication list, we can make sure that none of your medications have the potential to interact with each other. These interactions could be minor or could cause you to feel effects. It is also important to know what medications (including vitamins, herbals, and over the counter medications) you take at home so if your provider needs to prescribe a new medication, we know if there are any of your other medications that may interact with the new one.

What can I do to make sure my list is up to date? Bring your medications, ALL medications, with you to every appointment here at the Vanderbilt Comprehensive Care Clinic. If, for some reason, it is not possible to bring your medication bottles, a detailed list of your medications will work also. This list should include the name of the medication, the dose of the medication, how often you take it, and who prescribed it for you.

What if I am having problems with my medications? The VCCC has a new team of individuals who are here to help with medication issues. The Clinical Pharmacy Services team can help with medication reconciliation, education about medications, assisting in times when you may be having problems taking your medications, or if you are struggling with your daily routine of taking medications. Our goal is to help you be successful and as healthy as possible.
