How do I get a copy of my medical record?

To request a copy of your medical record:

1. Click here to download an authorization form.
2. Complete the form and mail or fax to:

You can also get copies of your medical records in person from
8 am - 4:30 pm Monday - Friday at:

  • Adult Patients: Room 2203 in the hall between Vanderbilt University Hospital and The Vanderbilt Clinic
  • Child Patients: Room 1025 Children's Hospital Admitting Department
